Please bring the following to your appointment:

 

• Current insurance card

• Any glasses you currently use, including sunglasses, computer, and/or near vision only

• If you are a contact lens wearer, please bring in your current pair of contacts along with the boxes or previous contact lens prescription

• List of medications 

• Previous eye health records 

• Face coverings / masks are now optional at our office for our staff, patients, and visitors.

• If a phone interview or arrangements for paperwork were not performed prior to your visit, please complete the appropriate questionairre below and bring it with you to the appointment. Thank you!
