What is RevolutionPHR? 


PHR stands for “Personal Health Record” and is your link to an online portal where you and only you can view important information about your relationship and patient history with us.


RevolutionPHR utilizes the highest standards of online security while allowing you to view portions of your eye care record through any standard Internet browser on a PC or Mac computer or iPad. We encourage you to use the login information created specifically for you to review the many features available.


You can decide if access to RevolutionPHR is right for you, and participation is completely optional. We have no record of your access and we allow you to use this online portal at your discretion.

Please bring the following to your appointment:


  • Current insurance card

  • Any glasses you currently use, including sunglasses, computer, and/or near vision only

  • If you are a contact lens wearer, please bring in your current pair of contacts along with the boxes or previous contact lens prescription

  • Please download, print and complete our "History Questionnaire" and "Office Policies" at home so that you can take your time completing this information before you visit us. Once you've completed it, please bring it with you to our office. Thank you!

What can I do in RevolutionPHR? 


  • Share with our office changes to your contact information, such as new phone numbers, email addresses, mailing addresses, and more.

  • Complete your pre-exam interviews online. This information is transferred to your patient chart in our system, leading to a more accurate and efficient exam.

  • Schedule appointments any time of day.

  • Check the status of glasses and contact lens orders.

  • See billing statements.

How do I access RevolutionPHR? 


When you register as a patient with Ithaca Eye Care, you may ask a staff member for a user name and password to start your RevolutionPHR account.


Family and individual accounts are available. If you are a parent logging in for the first time for a child's account, please use your own login information to access the family account. If you previously had individual accounts for each family member, and would like to combine these, please call our office so that we can reset your accounts.
